Application Checklist

All these attributes mean that our graduates leave with more than a degree. They leave with the knowledge that they have achieved academic excellence.
In order to be complete, the application packet you submit must include:
- Completed and signed application form
- $50.00 non-refundable application fee, check made payable to: SUNY Fredonia
- One official transcript in a sealed, signed envelope from each college or university you have attended. You must request of each college that the transcript be sent to you directly in a sealed, signed envelope. For your convenience, transcript request forms are available for you to send to your higher education institutions. The forms instruct the institutions to assist you and the Graduate Dean's Office in this process. Upon receipt of all transcripts, submit the envelopes, unopened, with all other application materials. Opened materials will not he accepted. If you have completed any course work at SUNY Fredonia, the Graduate Dean's Office will obtain your
SUNY Fredonia transcript(s) through the Registrar's Office upon receipt of your
application pocket.
- Two recent letters of recommendation, returned to you in sealed, signed
envelopes. A confidential recommendation will only be accepted in a sealed
envelope with the signature of the writer across the seal of the envelope.
Enclosed are SUNY Fredonia Graduate Studies Letter of Recommendation Forms
for your use which explain this process to the writer. Recommendation materials
intended to be confidential will not be accepted if envelopes have been opened.
Please note: If you select to have a letter of recommendation written on a
non-confidential basis, the signature of the writer across a sealed envelope is
optional.
- A copy of your current resume and a Statement of Intent explaining your
interest, what your hopes are, and why you selected SUNY Fredonia.
