Eligibility Requirements

• New York State Resident
  
• 7th through 12th grade student
  
• Academic potential in high school math or science
  
• Native American Indian, Alaskan Native, Black, Hispanic, or Economically Disadvantaged
  
• Reside in Gowanda, Lake Shore, Silver Creek and/or Salamanca School District, or on Cattaraugus/Allegany Reservations
  
• College-bound with an interest in a career in science, technology, health, or a health-related area
  

Admission Procedures:

• Email Application Form
  
• Parent/Guardian signature for release of information
  
• Social Security Number or Tribal ID
  
• Income Documentation required for economically disadvantaged only (NYS Guidelines)
  
• Upon receiving application and income documentation (if needed) a letter to approve or deny will be mailed out
