Requisitioning Instructions

1. Fill in completely all information called for by the various headings on the form.

2. Give special attention to columns 1, 2, 4 and 5 - must be written exactly as listed in storehouse catalogue in order to take full advantage of the computerized inventory procedure.

3. After printing the requisition, mail the requisition in duplicate to the storehouse. One of the copies will be returned with ordered supplies and/or an explanation for any changes made to the requisition as submitted.

4. Requisition that do not comply in full with the above will be returned for correction.
