Multimedia/Classroom

This form may be used by Faculty or Student. Student must get a Faculty signature.
Multimedia Equipment Request Form

Use this form for TV/VCR, DVD Player, LCD Projector, Computer on Cart, SUNYSAT / Teleconference, Slide Projector, Overhead Projector, Cassette Recorder, Screen, Flipchart / Easel

Video Camera/Audio Recorder
This form may be used by Faculty or Student. Certain video/audio equipment (marked with an *) require enrollment in corresponding courses. Overnight use requires a Faculty signature on a separate "Overnight Equipment Checkout Permission" form (yellow).
