Welcome to the LoGrasso Health Center Online Student Health Access Prior to the first day of classes we need you to complete certain Health Certification forms in order to comply with New York State law, protect your health and that of your fellow students, and have a basis for understanding and serving your health needs while you are here. Please read carefully as some forms can be completed online, and others will need to be printed out and either mailed to (SUNY Fredonia, LoGrasso Health Center, Fredonia, NY 14063 ) or faxed to (716-673-4722). Forms needing to be completed are: 1) Online Health History Form In order to begin submitting the information electronically for the Health History Form, you will need your E-services username and password. This user name and password can be set up through "Your Connections". Once you have your E-services username and password, please click on the following link to begin entry of your information into the online Health History Form. https://fredonia.medicatconnect.com/ Once you have successfully entered the portal, select “Forms” from the menu bar on the left and follow the instructions. The only form that needs to be completed prior to attending classes is the Health History Form. The remaining forms which are listed online are used for when students visit the health center. While working in this online system, please complete the Health Insurance information Entry (Found as a separate tab on the left column) as well. You are not required to have Health Insurance, but it is recommended. 2) Please mail or fax a copy of your Immunization Record . This record MUST include proof of protection against measles, mumps and rubella and a recent PPD within the past 12 months. If the PPD is positive, a chest x-ray done in the USA is required. Please include a copy of the chest x-ray report. 3) If you are an Incoming Athlete, you will need to have a physical examination ( SUNY Fredonia Athletic Physical Form (PDF) ) completed by your primary care physician. Once completed the form needs to be mailed or faxed to our office prior to participating in the sport. 4) If you are under the age of 18 years upon beginning here at SUNY Fredonia, you will need to complete the Consent for Medical Care for Minors form ( Consent for Medical Care for Minors (PDF) ) , and mail it to the above address.
