Prior to registering for classes, we will need you to complete certain Health Forms in order to comply with New York State law, protect your health and that of your fellow students, and have a basis for understanding and serving your health needs while you are here.
Please click on and print out the following links for all of the materials that you will need to complete in order to be allowed to register for classes.
When sending these materials back to the Student Health Center, we will also need a Copy of Your Immunization Record. This is very important as it is what the New York State Department of Health requires us to keep on record in the Student Health Center on all of our incoming students.
