Dear Incoming Fredonia Student and/or Parent,
The Student Health Center at the State University of New York at Fredonia provides currently enrolled Fredonia students with confidential primary care of acute illness and injury, health education, immunizations, preventative health and laboratory services. Our mission is to enhance the educational process and personal development of the Fredonia student by providing quality health care and promoting an optimal level of health and wellness in a caring atmosphere that respects their unique qualities as an individual.
Prior to registering for classes, we will need you to complete certain Health Forms in order to comply with New York State law, protect your health and that of your fellow students, and have a basis for understanding and serving your health needs while you are here. In accordance with New York State Public Health Law 2165, Fredonia requires that all incoming degree-seeking students (undergraduate and graduate) born on or after January 1, 1957 must provide proof of immunity against measles, mumps, and rubella. The state also requires that all students receive information about meningococcal disease and have made an informed decision about whether or not to receive immunization against meningococcal disease.

When sending these materials back to the Student Health Center, we will also need a Copy of Your Immunization Record. This is very important as it is what the New York State Department of Health requires we keep on record in the Student Health Center on all of our incoming students.
