Materials Request Forms
If the library does not have the materials needed for your research, they can be obtained from other libraries, generally at no charge. When there is a charge, we will contact you before ordering the material to see if you will be willing to pay for it. All borrowers pay the cost for overdue fines they incur. Interlibrary Loan request form for materials is available online. Patrons should contact Interlibrary Loan for complete details.
Use this form to recommend Reed Library purchase materials for the Library's collections. If ordering items to be placed on reserve for a course, please indicate in the notes field. For Departments: Please direct all questions about book allocations and ordering to Kathleen Sacco email@example.com. All other requests will be placed in a queue and will be assessed for purchased as funds are available. All materials are processed according to the Library's Collection Development Policy and Deselection Policy.
Have a list of items? Use this Excel Form to fill out the list and attach it to the form, rather than filling the form out for individual items.
