Forms and resources for planning performances and events as part of the School of Music.

Event Scheduling Request (ESR) - PDF form
Required to book/reserve any space in Mason Hall for a public event (concert, recital, master class, etc.). Return form to Marc Levy.


Guest Artist Information - PDF form
Must be completed if a guest artist is associated with your event (paid or unpaid). Submit as early as possible, but no later than 6 weeks prior to event. Return form to Jen Darrell-Sterbak.


Ticketing Service Request - PDF form
(Also available at the Ticket Office, located in the Williams Center)
Must be completed if your event is ticketed (whether free or charging an admission fee). Submit to Ticket Office as early as possible, but no later than 6 weeks prior to event.


School of Music Calendar Listing & Publicity - Online Form
To include a one to two sentence description for your event for weekly eNews messages, calendar listings and press releases. Email artist or PR images to Lori Deemer and Jennifer Darrell-Sterbak. Weekly eNews messages cover a rolling two weeks. Deadline for eNews inclusion is Thursday at 8am preceding the Monday distribution.


Public Relations (PR) Services - Campus PR Services online
To request publicity for your non-curricular event or newsworthy achievement (i.e. student group activities, faculty publications or performances that occur outside of the Fredonia campus.) Submit as early as possible (minimum 2-3 weeks lead time is required.) School of Music Printed Programs
School of Music printed programs for curricular events, faculty and guest artist recitals are formatted by Maureen Yuen. Deadline: 2 weeks prior to event; no late submissions are accepted.


Printed program guidelines:

AT LEAST TWO WEEKS PRIOR TO YOUR EVENT, email the following information to Maureen, unformatted, in ONE submission:
-event/ensemble name
-event date
-event time
-event location
-repertoire
-bios (all bios must total 700 words or less) and/or program notes (if necessary)
*if your program selections take up 2 pages, or a personnel list is included, the bios must total 350 words or less
-number of programs required
