What is an Affiliation Agreement?
An Affiliation Agreement is a formal contract between SUNY and an external organization (such as a hospital, clinic, nonprofit, or business) that allows SUNY students to participate in internships, clinical placements, or other experiential learning activities at that site.
These agreements ensure that:
- The responsibilities of both SUNY Fredonia and the host organization are clearly defined.
- Student rights, safety, and liability protections are in place.
- The arrangement complies with SUNY and State of New York contract requirements.
When is an Affiliation Agreement Required?
An Affiliation Agreement is required whenever:
- A student will be placed at an external organization for academic credit.
- The placement involves clinical, fieldwork, internship, or practicum activities.
- The host site requires a signed agreement before accepting SUNY students.
How to Request an Agreement
- Submit a Request
- Program coordinators should complete the Agreement Request Form.
- Provide Information
- Name and contact of the host organization.
- Student program/placement details (clinical, teaching, internship, etc.).
- Start and end date of the agreement.
- Review and Approval
- The Office of Contracts (or Office of General Counsel, depending on the agreement) will review the draft.
- Agreements must follow SUNY contract language requirements and cannot be signed by faculty or departments directly.
Processing Timeline
- Standard review: 2–4 weeks (depending on complexity).
- Agreements requiring negotiation with the host site’s legal office may take longer.
- Please submit requests well in advance of student placements.
Important Notes
- Only authorized signatories for SUNY Fredonia can execute agreements.
- If the external site provides its own template, it must be reviewed and revised to comply with SUNY legal requirements.
- No student may begin a placement until the agreement is fully executed.
- Agreements with other NYS Agencies require an MOU, not an Affiliation Agreement. Internship Agreements are used for paid placements.
- A COI can be provided for Affiliation Agreements, but not for Student Teaching for private schools.
