COVID-19 guidelines for Spring 2023

SUNY has mandated that all students be fully vaccinated 2 weeks prior to coming to campus.  Fully vaccinated means having received the last dose of vaccine 2 weeks prior to arrival to campus.  At Fredonia, new students can move on to campus January 19, 2023. Therefore the “due date” for all new students to submit evidence of full vaccination OR to have received an approved medical or religious exemption is close of business on Wednesday, January 18, 2023.


Vaccine timeline for a new student:

  • Pfizer or Moderna
    • 1st Dose: On or before Dec.12, 2022
    • 2nd Dose: On or before Jan.9, 2023
    • Send photo of Vaccine Card by Jan. 18
  • Johnson & Johnson
    • Single Dose: On or before Jan.9, 2023
    • Send photo of Vaccine Card by Jan. 18

How to submit vaccination/immunization records to the Student Health Center

  • Email a photo of your vaccination card and immunization records to health.center@fredonia.edu, or 
  • Fax to 716-673-4722, or
  • Mail to Student Health Center, SUNY Fredonia, LoGrasso Hall, Fredonia, NY 14063

What happens if a student fails to submit documentation?

Newly enrolled students who fail to provide evidence COVID-19 vaccination or who fail to receive an approved medical or religious exemption by close of business on January 18 will be deregistered from SUNY Fredonia and will not be allowed to move into the Residence Halls on January 19.


What if a student wants to complete a medical or religious exemption form?

New students can download and submit a Medical or Religious Exemption Form from the Student Health Center website.  Students must have received an approval from the Student Health Center before close of business on January 18, 2023. 

If a transfer student had an exemption at another school, they must still complete the form at Fredonia and seek exemption here.


What if a student indicates they are only taking “online” classes?

New students can download and submit an Online Only Exemption Form from the Student Health Center website.  Students must have received an approval from the Student Health Center before close of business on January 18, 2023.

If you receive a COVID-19 positive result or test positive via an at-home test (or receive an expectation to quarantine due to exposure) DURING Normal Business Hours (8am - 4:30pm M-F):

If you receive a COVID-19 positive result or test positive via an at-home test (or receive an expectation to quarantine due to exposure) AFTER Normal Business Hours:

STUDENTS:

  • If you are a residential student, notify the RD on Duty (716-679-8524) immediately
  • If you are a non-residential (off-campus student), send an email to health.center@fredonia.edu to notify campus of the requirement to isolate/quarantine - please provide your cell number in the email

EMPLOYEES:

  • Notify Human Resources by email: human.resources@fredonia.edu (non-state employees should notify their human resources director / supervisor)
  • Notify your supervisor per your unit/department procedures
